Definition of Care coordination is the organization consists of more than two people to serve quality patient care services facilitating appropriate delivery of health care to patient.

Care coordination refers to the delivery of patient care working as a team to meet and sort the problems ornillness of the patients. When there is more than one participant in the patients care improves the quality of care or there will be a multidisciplinary team of medical professionals in treating the patients.It can be categorised into inter and intra care.

The internal care coordination team plays an important role in the patient care.for example if a patient suffers Addison disease in a child ,here multiple professionals like endocrinologist,pediatrician,Nutritionist,cardiologist, Nephrologist,Urologist taken part in the treatment process by coordinating the types and needs of care.

The internal care coordination is important in the following way
