Both the approaches are unfair and non ethical in practice . Be it a non profit organisation or a patient with an insurance or an uninsured person the charges for the services should be same . By charging different from the patients it's breeching the ethical principle of justice - the principle of treating every individual the same way .

If I would be the ultimate authority such situations ,then the people who are playing behind the scenes will be caught first by conducting a survey of this malpractice and encourage patients to be aware of the charges , services they get so that they are not taken advantage of . Also the people who are found as the culprits would be fined and punished for the malpractice they did .
