As a student nurse, I will utilize ISBAR in my clinical experiences this mnemonic is created for improving the safety in the transfer of critical information; ISBAR (Identify, Situation, Background, Assessment, and Recommendation). For example: identify the Patient’s MRN, Name and DOB, Name and title/role of staff handing over. Situation: for example; the reason for admission (e.g. Hyperemesis @12 weeks). Background; relevant previous history (Admitted 25 days back with hypertension).Assessment; latest clinical assessment, clinical & investigation; BP was high, pulse normal and ECG normal.
