Convert 4 lb to kg.

lb → kg    ( ÷ by 2.2 )

4 lb ÷ 2.2 = 1.8182 kg

1.8182 kg * 1.5 mg/kg = 2.7273 mg

2.7273 mg is ordered and the medication is available as 75 mg / 3 mL.
