A patient calls the office asking for a new prescription for a narcotic medication that he has been taking for six months. You bring up his chart and notice that he has been requesting new prescriptions every 23 days, whereas the medication should last 30 days. Additionally, the patient also mentions that he feels that he is in need of a higher dose and gets agitated and irritable when you tell him that he will need an appointment. 1) What do you think of this? 2) What should you do?

1.Patient needs an appointment with doctor to change the dosage of the medication. Nurse has no rights to make changes in the doctor's prescription.

2.I will inform to physician and arrange an appointment for the patient to consult the concern doctor. Patient needs dosage changes because he not getting rid of symptoms and its getting more worsen.

  1. I think in this patient that it could be signs of addictions, side effects and dosage needs to be lowered or he may have toxicity, high tolerance! Non-judgement still call doctor and make the appointment necessary due to nurse's have no rights even by law to make any changes to any medications. 

  2. Tell the patient that you will inform the doctor and will arrange the appointment for the patient to consult the concerns. Patient needs dosages changes because he is not getting rid of symptoms and it is getting worse to the point of possible overdose and or addictions/toxicity/side effects.
