Actually it is basically the maintenance of proper medical records of the patients mostly electronically and to act depending on the data. WHO has the opinion for primary legal records documenting the healthcare services provided to a person in the healthcare system by Healthcare Information management.

WHO stated that "proper collection,management and use of informations with in healthcare systems will determine the health problems,identify solutions, priority selections and resource allocations to improve health"

So the mode of planning should be like this

1.Primary Record

Please prepare all the documentation like All Health Records of the patient in daily basis in the clinical history sheet of the patient given by HC Professionals e.g

a) Daily observation of patients Vitals and laboratory investigations or procedures adopted and

b) Actions taken by HC professional according to that

2. Secondary Record

A selected record derived from the above to help non clinical persons. That means to evaluate and support the various administrative processes e.g payment assistance etc

3.data quality should be maintained by following guideline

1.Valid Data which have correct values so that it can be properly analysed

2.Data should be easily accessible and no ethical problem should be there for collecting it

3.Comprehensive ,easily accessible reliable and up to a specific point of time collected data

The data should be Precise and at a Correct level of detail so that to support the applications and procedures taken depending on that data
