A.The 90% effacement indicates that the cervix is fully effaced or thinned and the baby has entered there.At this time the vaginal should be dilated and the station should be +4 or +5 for delivery but still it is in +2 station it means the baby has not descended .This may be a sign of fetal disposition and takes time to decent besides contraction .The decreased heart rate less than 100beats/minute and a deceleration indicates the baby is experiencing  a cord compression.

B.The immediate action to be done by the nurse are

  • Monitor vital signs of mother as well as fetal heart rate
  • Inform the concerned obstetricians or on duty doctors immediately
  • Change of position
  • Stopping oxytocin drip as per order to stop contraction and slower the baby's distress
  • Administer oxygen to prevent complications
  • prepare the mother for an emergency LSCS
  • Get an informed consent ready
