Ivy a 26-year-old G3P2, has been in labor for 6 hours and the cervical exam is 7 cm, 90% effaced and +2 station. The baseline fetal heart rate by external fetal monitoring is 120 bpm, minimal variability, and occasional variable decelerations. Just as a contraction is beginning to subside, you note a quick drop of the fetal heart rate down to a low of 80 bpm and a slow return to the baseline 1 minute after the contraction has ended.

What is the significance of this finding?
Discuss the RN management regarding fetal assessment and interventions that the RN, may implement or recommend at this time? (Utilize SBAR communication)
1. Fetal heart rate is decreasing means deceralation of fetal heart rate due to uterine contraction . Details is unable to bear the uterine contraction and as a result heart rate is decreasing.
2. The significance of this finding is that heart rate is decreasing and and the foetus is having distress . Immediate delivery of the baby is needed.
3. The intervention that the RN must implement are :-
- should inform the physician immediately
- interventions to decrease the intensity of uterine contraction until fetal distress is relieved
- continuous monitoring of fetal heart rate
- preparation for the delivery of the baby
