Data are individual units of information.Data is distinct pieces of information usually formatted in a special way.A data describes quality or quantity of some object or phenomenon.

Data collection mainly includes the demographic details of the patient,social background,disease condition,past medical history,medication history etc.

Data collection methods;

There are two types of data collection methods are there,

PRIMARY DATA COLLECTION;Data collected directly from the subjects or researcher by asking questions that is direct interview. eg; direct patient interview

SECONDARY DATA COLLECTION;It uses the information from already existing data.Here the patients previous health records helps to collect the data.

DATA ASSESSMENT; Assessment of data is the gathering information about a patient's physiological,psychological,sociological and spiritual status.NURSING ASSESSMENT IS THE FIRST STEP IN NURSING PROCESS.Nursing assessment used to identify current and future patient care needs.

CLINICAL REASONING AND JUDGEMENT;

Clinical reasoning and decision making are the thinking processes and strategies we use to understand data and chose between alternatives with regard to identifying patient problems in preparation for making nursing diagnosis and selection of nursing outcomes and interventions.

Beyond observation clinical judgement allows the nurse to associate pieces of information to review them,to establish relationship with known facts and to analyze and interpretate the data at hand from both a critical an rational perspective.
