A discharge plan has been prepared for a chronically ill 60-year-old male (Shaun) hospitalized after falling and fracturing multiple limbs in his home. He needs assistance with ADL activities, mobility, and medication management. The health care provider orders services of nursing, physical therapy, respiratory therapy, social work, and home health aide. After two weeks, the spouse indicates that they are dissatisfied with the services of the home health aide and the physical therapist.

ANSWER :

HOME HEALTH AIDE : Home health aide persons assists the patients to perform their daily living activities and helps them to perform routine activities like brushing of teeth, eating, vitals checking, medication administration, and helps the patients with walking aids.

PHYSICAL THERAPIST : Physical therapists are the persons who are licensed health care providers with highly educated experience, who helps patients to decrease pain and sufferings, restoring of body mobility without any use of surgical management along with decrease the usage of using long term medications there by prevents their side effects on patients health.

Registered Nurse (RN) has to deal with these health care providers in order to establish a quick and healthy recovery of patient from pain and sufferings. The following measures or steps can be taken by the nurse in dealing with the home health aide and physical therapist :-

* Identify the health care providers selected for providing care to the patients

* Explain both the health care team about the heath condition of patient and his age preference

* Discuss the plan of action with both the health care departments

* Identifying the goals of patient care and services that had been providing by the teams

* Act as a coordinator between the home health aide and physical therapist

* Planning the time arrangements for both to prevent clash in providing their health services

* Plan a appropriate home settings which promotes the services to perform effectively

* Discuss the problems of both teams and try to solve those problems at that point of time

* Encourage for a positive feed back from patient

* Evaluating of the care provided to patient and take response from him

* Effective management, coordination, planning, responsible towards the health care team members can improve the services to the elderly and develop a positive response from patient.
