1. Shaun's is an elderly patient with multiple limb fracture needs assistance in everything. The main goal of home health nursing is to improve the quality of life, provide a positive atmosphere, ensure patient safety and make them comfortable in the home environment. Home health nursing is one on care services. Care is provided by coordination of team members such as a nurse, physical therapist, social worker, home aide altogether. Irresponsibilities of one member of the team would spoil the entire patient care. As a Home Health RN, she has responsibility for the entire team. Failure to do their duties of the team members would be identified and to be warned. Physical therapist and home aide should teach of their roles and responsibilities and their influence in the patient outcomes. Still, they become irresponsible to their duties, then RN should report to the administration and replace with other personnel.

2. The common roles of home health nurse RN are

  • Plan adequate care for the patient
  • Supervising the duties of team members.
  • Support them in resolving issues.
  • Engaged with patient advocacy of care
  • Talk with patient and family members and find the patient needs.
  • Identify the reason for the lag in patient care.
  • Evaluate patient condition and outcomes.
