Beth is a 31 year old Nurse Practitioner G3 P2 with a 9 year old daughter and an 18 month old son. Her first labor was uncomplicated, but during her second the fetal heart rate showed late decelerations. After delivery, her placenta was examined and found to have part of it missing. Beth underwent a dilation and curettage (D&C) to remove the piece(s) of placenta that were retained in her uterus. She was discharged home uneventfully.

Beth presented at 34 weeks gestation with some mild bleeding. After taking vitals, monitoring the baby and doing tests, the provider told Beth that she has mild hypertension and placental abruption. She was admitted for a week, monitored, then went home on bedrest. Her blood pressure remained mildly elevated.

She returns today after her water broke. She is given oxytocin after she fails to start contracting on her own. In addition to pain from contractions, she is experiencing a headache. She is given an epidural. Labor progresses, then she meets her healthy baby boy of 9 lbs 5 oz. She marvels at him and asks if she is still bleeding. Then everything fades away.

Beth went into postpartum hemorrhage (PPH).

What were some of the early warning signs that Beth demonstrated for PPH? (Do not copy and paste)

What are the risk factors for postpartum hemorrhage? (Do not copy and paste)

Which of those risk factors did Beth have?

Discuss how you would explain what was happening to Beth's husband and how you would include him in the emergency care for Beth.

What else (other condition) are we worried about in Beth's case?
