***How do i help this patient in this situaton? need help

Sarah Grace is a 40 years old woman who came for consultation for the first time. She said she moved from Texas to live with her ailing mother who is 76 years old. It is uknown if she had previous prenatal visit but stated she is 32 weeks pregnant.

She appears quiet and reserved about her financial status but only says she previously worked as a grocery cashier. She indicated that she had spontaneous abortion 2 years ago and had previous abortion when she was 15 years old.

She does not indicate if she is married but seemed to be avoiding discussion about her support system.

She is asking questions about how to know if she is in labor and how to prepare for it. Her current health status needs to be assessed. You are going to meet with the patient prior to a doctor interaction. A health history is not available or any previous lab work.

Gather pertinent information that will aid the doctor in helping Sarah Grace. Please be keen on the needs of the patient that may present during your initial interaction.

The CNA had taken her vital signs as follows:

The patient has to be communicated by the nurse properly in order to get relevant information regarding her maternal health. Reassure the patient, so that the patient's trust can be gained. Patient's past medical history should be thoroughly collected so that maternal and fetal complications can be minimized by following appropriate intervention priorly. Patient current gestational age should be estimated accurately and to evaluate the maternal and fetal status.

Assess the patient's weight gain throughout the pregnancy and follow necessary interventions on the targeted weight gain. Assess the lab values for anemia and ensure the intake of folic acid and iron tablets. If not the patient has to follow with specific iron medications as prescribed by the doctor. Patient blood pressure should be evaluated thoroughly and controlled with proper medication. Because high blood pressure increases the complications during delivery. Patient already had a history of abortion and advanced maternal age increases the risk of chromosomal abnormalities, low birth weight, and preterm babies. This can be avoided through the regular follow up of visits and monitoring of fetal growth.

The patient has to be educated with nutritious diets such as folic acid, iron, calcium, vitamin, and minerals. Discuss their lifestyle habits and explain the risk of intake of alcohol, tobacco, and other drugs. Explain the importance of weight gain and muscle strengthening for a healthy mother and baby. Explain about the prenatal testing and testing for chromosomal abnormalities to avoid deformities in the fetus. Discuss the financial status and give information about federal agencies such as WIC, SNAP, CHIP, which support the pregnant mothers in many ways.
