three mechanisms of action of antidiarrheals:

  • Absorbants

these type of antidiarrheal agents can slow down intestinal contraction and decreases the frequency of bowel excretion. This enables the absorption of water from the intestine and reduces water content in the stool, bulking up the stool,but not reduce the water and electrolyte loss.

example: Isphagula, methylcellulose

  • Antimotility drugs

it reduces the propulsive movement of the intestine and decreases intestinal secretion by increasing absorption.so it gets more time for absorption.

example: codeine, loperamide

  • antisecretory drugs

these drugs inhibit cytokinins and interfere migration of inflammatory cells into the bowel wall with reduced mucosal secretion. so if it is given during an exacerbation it reduces the quantity of excretion and reduces abdominal cramps and fever due to diarrhoea.

example: sulfasalazine,octreotide
