James a 52-year-old Hispanic male, had a routine screening colonoscopy last week. He has a history of smoking cigarettes for 40 years, hypertension which is controlled with lisinopril, and type 2 diabetes which he take metformin daily. He was asked to come into office for the results this morning. Mr. James was diagnosed with colon cancer and is scheduled for surgery to remove the tumor in two days. The plan after the surgery is to provide radiation therapy for the patient.

This surgery was performed with collaboration of other experts of healthcare professionals as the patient was having diabetes and the post operative care is necessary.

Mr. James was given discharged teaching such as taking healthy food as well as he should quit smoking immediately.
