Answer #1

No 1. Directions for the reconstitution of ampicillin sodium for injection For IM use add 3.5 mL sterile water for injection.

The vial contains ampicillin sodium equivalent to 1 gram.

After adding 3.5ml the solution contains 250 mg ampicillin sodium per ml.

The usual adult dosage of ampicillin is 250 mg–500 mg IM every 6 hours.

Then you have to administer 1 to 2 ml of the drug.

3.5 ml of diluent is added to the vial to prepare the medication for IM use.

No 2. Order Dose 10mg/kg

the child weighs 18 kg

So, the total dose will be 180mg (10mg x 18kg = 180mg)

The oral suspension is dispensed as 200 mg in 5 mL

I.e 1 ml contains 40mg (200mg + 5ml = 40mg)

We have to administer 4.5ml of oral suspension. (180mg = 40mg/ml = 4.5ml)

Answer - 4.5ml

No 3. Directions for reconstitution of oxacillin sodium for injection For IM use add 5.7 mL sterile water for injection.

After adding sterile water the solution contains 250 mg oxacillin sodium per 1.5 mL.

The client is to receive 400 mg IM every 6 hours.

250mg + 1.5ml = 166mg, I.e 1 ml of solution contains 166mg of oxacillin sodium.

Then 0.1ml contains 16.6mg of oxacillin sodium.

So, we need to administer 2.4ml to the client.

No 4. Order states administer 1000 ml D5NS IV from 4 am to 4 pm using a macro drip (10 gtt/ml)

1000ml in 12 hours.

Drip factor formula is Volume x Dripfactor - TimeIn Minutes

1000ml x 10gtt/ml = 720min = 13drops/min

You have to administer 13gtt/min
