Answer: Nursing intervention for wound infection:

Diagnosis: The diagnosis of the wound reveals about the large grayish colonies non pigmented, soft and smooth.

Outcome: Reduce the spreading of the infection to other body parts. Reduce in the symptoms.

Intervention: Nurse should intervene about the wound and the color of the wound grayish showing presence of fluid in it. Avoid moisture contact to the wound.

Rationale: The moisture won't lead the wound to heal and the presence of fluid increase the spread of infection in more areas. Make the area dry as well as reduce the contact with moisture.
