If client is saying that she is always dieting but not losing weight.The Nurse must ask regarding her diet plan that which type of diet she is taking , on what time and how much calories? Because there may be possibilities of lack of assistance or awareness regarding :

  • May be she is losing without realizing it because it's a slow process where body weight tends to fluctuate by a few pounds after 2-3 weeks.

  • May be she is not cutting back on carbohydrates or Low protein intake may bring her weight loss efforts to a standstill. Make sure to eat plenty of protein-rich foods.

  • May be she is not doing exercises and also taking sugar from another sources like beverages that can be healthier but contain sugar in it.

  • May be she is not taking sufficient water or not eating mindfully etc.

Hence all above reasons can lead her diet plan less effective that nurse can come to know after collection of nutritional history of client.
