1. Imbalanced nutrition less than body requirement related to insufficient intake of food due to fear of cs delivery .

Subjective data_i don't have appetite while thinking of ceaserian section deliveryas verbalised by the patient.

Objective data-patient seems pallor and weaK

Rationale -reduce anxiety to a manageablelevel and improved her health to a normal level .

Objective - maintainace of patientsand her gestational baby's safe by relieving her anxiety and improved nutrition in her diet therby improved her health.

Planning -give health education regarding antenatal requirement of nutrition for health

.,explain about seriousness regarding nutrional requirement for mother and baby's safety .

Relieve her anxiety regarding delivery .

Scientific rationale -a.provide psychological support ,and to reduce fatigue

Minimise anxiety

To promote health

Evaluation -patient will be able to take food  

2.Anxiety related to hospitalization and delivery process

Subjective data -client verbalised concern about upcoming delivery especially CS and express worried about that.

Objective data-facial tension observed, impaired attention noted

Rationale - within our care patient will acknowledge and discussfears recognizing healthy Vs unhealthy fears.

Planning - health education should give about value of child birth and life of a baby., explanation to patient regarding hospitalization and delivery process  

Instructions should give about diet and medicines

Interventions -health education about delivery relieve anxiety to a great extend. Explanation regarding value of baby will ease out tension.

Proper intake of diet and medicine help to improve health .

Evaluation - patient anxiety relieved by assessment.
