1. SUBJECTIVE DATA ;- Patient said that "I have lost appetite because, I am affraid to eat a lot because it might lead to a possible CS delibery."

OBJECTIVE DATA ;- Pale looking

                              - Generalised weakness observed

2. NURSING DIAGNOSIS ;- Knowledge deficit related to lack of information about pregnancy care diet and nutrition.

3. RATIONALE ;- cognitive impairements must be recognised so an appropriate teaching plan can be outlined.

-interventions need to be specific to each patient considerng their individual differences and their backgrounds.

4. GOALS AND OBJECTIVES;-

-Patient explains dietary patterns during pregnancy

-patient shows motivation to learn.

-patient idenifies learning need

- patient lists resources that can be used for more information or support after discharge.

5. NURSING INTERVENTIONS;-

-Include the patient in creating teaching plan , begining with establish objectives and goal for learning at the beginning of the session.

-consider what is important to the patient.

6. SCIENTIFIC RATIONALE FOR THE INTERVENTIONS;-

-Goal settings allows the learner to know what will be discussed and expected the session.

-Allowing the patient to identify the most significant content to be presented first is the most effevctive.

7.EVALUATION CRETERIA;-

-Patient expalains pregnanacy dietary plan.
