NCP- RISK FOR LOW BIRTH WEIGHT BABY ;

5 SUBJECTIVE DATA AND OBJECTIVE DATA;-

1) SUB;- Patient said that " I have fear to eat a lot because it might lead to a possible CS delivery".

OBJ ;- Observed her facial expression shows fear.

2) SUB:- patient verbalised "she has generalized weakness".

OBJ ;- she is pale and weak.

3) SUB;- Patient said that "I have recently lost appetite"

OBJ ;- General body weakness present.

4) SUB;- Patient verbalized "I have no knowled about pregnancy care diet and nutrition."

OBJ;-Verbalizing inaccurate information .

5) SUB;- ( Twin pregnancy leads to risk for low birth weight baby). USG reprort shows multifetal gestation.

OBJ ;-fetal heart rate monitored.

NURSING INTERVENTIONS AND THEIR RATIONALE ;

- Determine level of client's knowledge. ---->Provides opportunity to clarify what has been learned previously, to identify cultural myths, and to correct miscoceptions.

-Review history of previous pregnancies ----->To check presents of complications.

-Obtain history about prenatal screening and amount and timing of care ----->Lack of prenatal care can place both mother and fetus at risk.

-Insruct the patient to take more well balanced diet ----> lack of dietary knowledge of during pregnancyn can leads to both mother and fetus at risk.

-Provide information and assist in ultrasonography as indicated ----->Detects presents of complications that may affect pregnancy.

-discuss implication of preexisting condition and possible impact in pregnancy----->Pregnancy have no effect , ormay reduce orrexacerbate severity of symptoms of severity of chronic conditions.
