Evaluation

Subjective:I am afraid to eat a lot because it might lead to a possible CS delivery” as verbalized by the patient.

Objective: “Restless Narrowed focus voice Quivering, Hand tremors, facial flushing, sweating.

Anxiety regarding risk for miscarriage as evidenced by increased tension.

Long term goal: After series of nursing intervention the patient anxiety will be eliminated.

Short term goal: After 10-30 min of nursing intervention the client will able to know techniques on how to lessen the anxiety such as deep breathing exercise and verbalization.

Assess patient level of anxiety.

-Monitor vital signs.

Acknowledge awareness of patient anxiety.

-Instruct to do deep breathing.

-provide accurate information about her present condition.

Different level of anxiety will affect the coping mechanism of the client.

-To identify physical responses with both medical and emotional conditions.

Acknowledgement of the patient‘s feelings validate the patient feelings and communicate acceptance of those feelings.

-This may help the client to relax.

-Helps the client to identify what is reality based.

After 4 hours of nursing intervention, patient has relaxed and anxiety reduced as evidenced by voice Quivering, Hand tremors, sweating and verbalization.
