MOST LIKELY REASON FOR AN INCOMPATIBLE UNIT OF BLOOD BEING TRANSFUSED INTO A PATIENT :

  • Most of the blood transfusion mistakes is due to the different Rh factor, if the Rh factor is +ve and is tranfused into the patient with -ve Rh factor it can lead to hemolysis which is a serious side effect.
  • Staff negligence towards the checklist before administering blood to the patient.
  • Patient with the same name or surname could be transfused wrong blood.
  • Blood bank staff mismatching the blood sample of patient can also lead to transfusion of incompatible blood.
