Mrs. Jones and her family have been patients at Family Medical Group for the past 10 years, since her family moved from Puerto Rico. Her youngest daughter, who lives near her parents and often acts as her translator, drove her to your clinic today because she is not feeling well.  

Mrs. Jones is a 63-year-old, obese (250 lb) female with a history of diabetes for 10 years, high blood pressure (130/90), osteoarthritis in both knees, and presents with one week of mouth pain and swelling in the left upper jaw. In addition, she has had several episodes of feeling dizzy and sweaty during the day. When she checks her blood glucose levels they have been low – in the 50’s or 60s (normal around 100), at which time she drinks some juice or eats a candy bar and the symptoms resolve.

She was prescribed an oral medication twice daily for diabetes, oral medication once daily for high blood pressure, an antidepressant she takes PRN (as needed) when she feels depressed. Additionally, she has been taking Nyquilmost nights to help her sleep because she is very anxious. The patient states she has not been eating or drinking well for the past 2 months due to pain, dry mouth and loss of taste. She also states that she has been too tired and has too much knee pain to go shopping. Her daughter confesses that her mother and father have been eating mostly fast food and/or pre-packaged foods for dinner which others bring to them.   

Further, she has not been to see her primary care physician or dentist for nine months due to difficulty with community ambulation and limited access to transportation. 1) Mrs. Jones is a 63 year old lady with the diabetes ,high blood pressure and osteo arthritis .

• She is very depressed due to no one present with them in their house and she is very anxious about her health .
• Mrs. Jones health is worse and she is unable to drink or eat properly due to mouth pain ,so she is getting hypoglycemic effects frquently .
• The financial support also not there For them and insurance also lossed .so she is feeling anxious For high health care cost and who will give money For her health issues and again it was burdon For her .
• She wont have any transportation facility For easy access to health facility.
• So ,the home  health care very beneficial For the mrs.jones .in home care ,health care providers will go to patient home and provides quality care who are unable to access health facilities .
• She need financial support For her treatment .
• Family support also needed .

2) home care is needed For Mrs. Jones ,so home health care providers needed to provide care to Mrs .jones .

Health care team work collaboratively to decrease mrs. Jones suffering .

Effectuve communication very beneficial in this care

Staff should maintain good IPR with patient to reduce her depression and anxiety.

She needs rest and comfort and peace ful environment to control her BP

She needs effective dental care to reduce gum problem and to reduce swelling .
