Answer: Before prescribing any medication the Healthcare professionals inquires about the the medical history. This is to know about the any kind of medication the patient is taking or has taken. This information helps the healthcare professionals to decide which and what kind of medication should be given to the patient to avoid any interaction or side effects.

If patient has taken any medication prior to the assessment then the result will be altered and the right medication and effect will also differ. For example if patient has taken antihypertensive drugs before the assessment and when nurse assess the blood pressure, she finds it normal. If the medical history were inform to the healthcare professionals then he will not provide medicines for hypertension although the patient is suffering from hypertension.
