1. Here the patient who is diagnosed with Acute Lymphoid Leukemia may be while on chemotherapy, there may be a high risk for getting an infection. And if develop a fever of 100.8oF indicates of infection. As soon as possible notify it to the physician and assess for any shaking chills. and check for the WBC. With physician instruction, she may be held if the WBC count is too low

2. In the case of type 1 diabetes mellitus, the patient needs continuous insulin therapy to control their blood glucose level. Here in the capillary blood test, the patient shows a high blood sugar level ie, 275 mg/dl that indicates the patient is hyperglycemic. The nurse should report it to the physician any unusual changes occur and if needed check the blood sugar level frequently and change the dose of the therapy as prescribed.

3. Dialysis, a procedure that uses a special machine to replace the kidneys in filtering waste from the bloodstream in End Stage Renal disease that may sometimes reduce the daily urine output that a person normally produces. This happens because as the blood is filtered during dialysis, fluid is removed, thus reducing the kidneys' traditional role. As a result, many dialysis patients produce very small amounts of urine. However, dialysis does not prevent someone from urinating normally; it only reduces the total urine output, so that he or she may only need to urinate once a day, which is not dangerous.

But if the patient is not in dialysis and there is less urine output for past hour the nurse has to report the physician soon that the patient is in a critical stage and need some management and as well as check for the physical characteristics of the urine also.

4.
