Lamotrigine:
Purpose of medication:
Expected pharmacological action:
It is an AED of the phenyltriazine class, Anticonvulsant and mood stabilizer
Therapeutic use:
It prevents seizure and extreme mood swings in bipolar disorder
Complication:
Unusual bleeding, stiff neck, vision problem, loss of coordination, weakness, muscle pain, and kidney problem
Medication administration:
Oral administration 25mg, 100mg, 150mg, 200mg tablets available as a maintenance dose.
Contraindication:
when a patient having anemia, low blood count, meningitis, less platelet count, less neutrophil, suicidal thought, depression, liver problem, CKD stage 4
Nursing Intervention:
-Watch the patient for side effects like skin rash, hives, blistering, peeling or mouthsore
- Educate the patient about valproic acid medication reaction
- Educate patient about side effects of medication
-Advice family to stay with the patient to prevent suicidal thoughts
-collect patient medical history regarding the liver disease, allergic reaction.
-If the patient is pregnant seek doctor's advice
-Ask the patient about if any birth control pill usage
-take the medicine correct time daily if you miss the dose don't take an additional dose
Interactions:
Drug interaction with alcohol intake and disease like depression, rash, arrhythmias, renal disease, meningitis, other drugs like aspirin Advil, amphetamine, diphenhydramine, fish oil, ibuprofen, lithium, paracetamol, tramadol, vitamin B12, vitamin D3. alprazolam, bupropion.
Evaluation of medication effectiveness:
Validating assessment is recommended by evaluating older and newer antiepileptic medication. The average dose of 200mg/day can control the symptoms and can be followed by a maintenance dose, this effectiveness can be evaluated by no weight gain or related metabolic abnormalities.
client education:
-Advice the patient if there is any skin rash stop the medication immediately
-report if there are any unusual symptoms
-Advice them to have a regular visit
-do not stop the medication without doctor permission.
-do not crush, chew or break the tablet.
