Expected pharmacological Action

It directly acts on the CMS and causes release of dopamine and norepinephrine which leads to stimulation of prefrontal area and the concentration is increased.When taken orally it is metabolized in liver enters the CNS and action takes place witha half life of 2_3 hours.It is excreted in urine.

Therapeutic use

  • Narcolepsy
  • Attention Deficit Hyperactivity Disorder
  • Bipolar disorder
  • Used in treatment of cancer to increase the analgesic effects

Medication administration

  • To be administered orally
  • Not to exceed 60 mg in a day
  • In paediatric case 5mg twice a day
  • In adults 20_30mg daily
  • If to be stopped shoukd be tapered to prevent reactions

Contraindications

  • Hypersensitivity
  • Patients having glaucoma
  • pregnancy
  • Women in fertility years

Interaction

The drug interacts the action of certain anticonvulsant ,anticoagulant and in depression tretament

Evaluation of medication effectiveness

  • Improved attention can be seen in the patient
  • Agitation are decreased
  • Will be able to concentrate

Complications

  • Increased rate of drug abuse
  • Cardiovascularproblems
  • Decreased growth rate
  • Cancer
  • If there is a history of using MAOI (Monoamine oxidase inhibitor ) in less than 14 days.

Nursing intervention

  • Monitor patients vitals
  • Observe for adverse reactions
  • Perform mental status examination

Client education

  • Inform and educate about the common side effects
  • Encourage patient to have a track of blood pressure and pulse and report immediately in case of chest pain or syncope.
  • If priapism occurs patient to report meducal team for help
  • Educate about the dependence or abuse in using this drug
  • Inform patient to hand over the unused medication to the medicine take back program
  • Encourage patient to do minute activity using fingers to avoid perioheral vasculopathy.
