Admission can happen in two situations one in emergency conditions and the other by referred by the consultant.
If in emergency:
The triage nurses should inspect.According to the triage code handle the situation.If the client is in Triage category 1 and 2 can get the necessary patient details from the care takers of the client.If he is conscious and stable then directly get information from the client.
If patient is referred by the consultant , the client will be stable so that get information directly from the client.
