Ans1) Albuterol

Class: Sympathomimetic bronchodilator

Indication:Albuterol is indicated for the treatment of bronchospasms associated with: asthma, COPD, allergic reactions, and toxic inhalation.

Major side effects:Tachycardia, hypertension, dizziness, tremors, headache, and arrhythmias.

Nursing consideration:

- pregnancy safety: category
- albuterol may precipitate angina pectoris and dysrhythmias
- albuterol should be used with caution in patients with dm, hyperthyroidism, prostatic hypertrophy, seizure disorder, or cardiovascular disorder
- in prehospital emergency care, albuterol should be administered only via inhalation

2. Apixaban

Class: Factor Xa antagonist

Indication:

• Non-valvularAtrial Fibrillation (Stroke prevention)
• Prevention of DVT/PE in Hip or Knee Replacement
• Treatment of DVT/PE

Major side effects:

- bleeding, nausea

Nursing consideration:

- predictable drug interactions (metabolised via CYP 450)
- 75% is metabolised by the liver, the rest being renally excreted
- no need for therapeutic monitoring
- currently no available antidote

3) Bumetanide

Class: Diuretic

Indication:

- CHF
- Acute pulmonary edema
- Edema unresponsive to other diuretics
- Patient allergic to furosemide

Major side effects:

1. DIZZINESS
2. Hypotension
3. MUSCLE CRAMPS
4. WEAKNESS
5. IMPAIRED HEARING
6. Hyoeruricemia
7. Hypochloremia
8. Hyponatremia
9. HYPOKALEMIA
10. Azotemia

Nursing consideration:

- Store in a cool, dry place away from sunlight and chlidren

- Monitor daily weight, I & O, vitals, skin turgor and edema for fluid volume status. Assess for volume depletion (dehydration): dizziness, orthostatic hypotension, tachycardia, and muscle cramping, thirst, rapid pulse. Monitor serum electrolytes & replace as indicated. Avoid night doses to ↓nocturia.

4) Bupropion:

Class: - Atypical antidepressant

Indications:

- diminish depression
- depression with seasonal affective disorder
- treat ADHD in adults

Off label: treat ADHD in adults, to increase sexual
desire in women and treatment of nicotine addiction

Major side effects:

CNS: SEIZURES, SUICIDAL IDEATION
CV: CARDIAC DYSRHYTHMIAS
GI: Constipation
INTEG: STEVENS-JOHNSON SYNDROME

Nursing consideration:

Hepatic/renal function
Increased risks of seizures
Mental status: suicidal tendencies, increased in psychiatric symptoms

Assistance with ambulation during beginning therapy because of sedation occurs
Safety measures, primarily for geriatric patients.

*Therapeutic effects may take 2-4 weeks
Use caution when driving, performing activities that require alertness
*Avoid alcohol--alcohol may increase risk for seizures
*Notify prescriber if urinary retention occurs
Suicidal ideas, behaviors, hostility, depression may occur
