Errors in medicine has led to an increase in mortality.most of the errors are preventable.causes of errors can be varied like personal cause,system or process cause,complexity in the treatment.errors committed by a doctor can have a very adverse effect on the patient .various types of errors are.

1.medication errors :doctors can commit medication errors due to wrong prescription caused by prescribing the wrong patient for a wrong diagnosis .when a doctor's writes drug order sheet for many patients at a given particular time he may mix the patients this can be prevented by maintaining all drug order sheets in individual patient file and use of patient identifiers .the other forms of medication errors are wrong dosing.The doctor may wrongly calculate the needed dose.

2.wrong surgery: The other type of doctor error is performing surgery on a wrong patient or at a wrong site .this is caused because of not following a patient identification checks prior to surgery.WHO recommends use of surgical safety checklist as a consolidated checklist to be done before surgery .This involves the entire surgical team like surgeon,anesthetist,nurses cross check the patient identification,site.Another way of avoiding wrong site is use of marking at the surgical site .

3.Misdiagnosis:Another major error area for doctors is missing diagnosing a patient this can cause serious effect as the treatment indented for a particular diagnosis may be harmful.this can be prevented by having a unit team of doctors for all treatment or diagnosis of patients.This will help multiple expert's to diagnose and check .

4.Wrong blood group transfusion:Doctor checks the blood before any forms of transfusion.So if the doctor is careless or have misread the information .it can lead to wrong blood group transfusion.This can be avoided by multiple checks by different personnels .

5.misdiagnosis a psychiatric condition:Many psychiatrist misdiagnose a patient like a patient having acclasia cardiac has excessive vomiting which can be diagnosed as buleimianervosa .
