Answer: Nurse plays important roles in patient transition as nurse make sure that the patients gets proper care and also removing the barriers between the one level of the care to the other level. The nurse aims to provide safe and specific environment to the patients so that they may feel more comfortable. The nurse also indulge the evidence based practice to bring out the best results by the treatment provided to the patients. The nurse regulate and monitor the level of the care that the patient required from the admission of patient in the hospital to their stay. The main purpose is to enhance the quality of care that is being provided to the patients.
