NURSING CONSIDERATIONS FOR A PATIENT ON MAGNESIUM SULFATE ;

Check serum magnesium level prior to administration.

Cardiac monitor should be used on patients receiving magnesium sulfate intravenously.

Have injectable form of calcium gluconate available to reverse paralyzing effects of magnesium sulfate.

Blood pressure may drop if magnesium sulfate administered too rapidly.

check blood pressure and pulse every 10-15 minutes during therapy.

Normal serum plasma level of magnesium sulfate is 1.8-3.0 mEq/L. cardiac arrest occurs at 25 mEq/L.

Assess patellar reflexes hourly to monitor for evidence of toxicity.

Monitor hourly urinary output.

Do not brrast feed while receiving magnesium sulfate sulfate.

SIDE EFFECTS:

Flushing, sweating.

Extreme thirst.

sedation.

confusion.

depressed reflexes, flaccid paralysis.

heart block, cardiac arrest.

NURSING ASSESSMENT;

During infusion; Vital signs, with specific focus on oxygen saturation and respiratory pattern, deep tendon reflexes, fuild intake/ output, topography, signs and symptoms of labour progression, worsening signs and symptoms of preeclampsia, magnesium toxicity and fuild overload.

MAGNESIUM SULFATE IMPACT ON LABOUR:

Magnesium sulfate can act as a tocalytic, it also act as an anticonvulsant, CNS depressant and it will be administered through a secondary IV line it can result in decreased DTR's and urinary output. calcium gluconate is the antidote for magnesium sulfate.

Some medications a patient with preeclampsia with HELLP syndrome might be getting are anticonvulsants, magnesium and CNS depressants.

Both mother and baby will need to be on constant monitoring.

Mother will need to be on seizure precautions and fall risk precautions.

An order of calcium gluconate STAT will need to be on hand in case of respiratory depression.

Other medications will be given to adress the hypertension like labetalol or hrdralazine.

Monitoring I/Os are also important as well as epigastric pain and neuro changes.
