1Alterations in Health

HELLP syndrome is a disease found in the advanced stage of pregnancy and the major alterations noted are mostly similar to normal pregnancy symptoms like nausea,vomiting backache but major alterations are severe right upper epigastric pain ,hematoma in the right abdominal region, hypertension.head ache associated with blur vision

2.pathophysiology

Pathophysiology in vascular system

HELLP syndrome is most of the time advanced stage of eclampsia or pre eclampsia . hypertension and hemoconcentration causes increased vasospasm and vascular construction mediated by prostaglandins ,enthlians which causes third space fluid shift causing edema .so a negative intake output to be maintained in essential

Pathophysiology in hematologic system :due to increased hemolysis caused by dysfunctional hepatic system hematocrit may be low along with thrombocytopenia.

Pathophysiology in hepatic system

Preeclampsia causes raised SGOT and SGPT liver enzymes .which causes hepatic capsule irritation and sometimes rupture which causes hemorrhage and hematoma associated with severe right abdominal pain

Pathophysiology in nervous system

Hypertension caused raised intra cranial pressure which can lead to seizures and coma

3.risk factors

Mothers who had earlier pregnancy with the same syndrome, hypertension,metabolic disorders, eclampsia,preeclampsia, overweight with higher BMI, Elderly gravida

4.expected findings

The patients will HELLP syndrome has hemolysis ,elevated liver enzymes and lower platelet count leads than 100*100*9 .deranged SGOt and SGPT liver enzymes,deranged INR,patient may also be

Disseminated intravascular coagulopathy.lower hematocrit due to hemolysis ,

Patient will have a systolic BP of above 160mmhg ,decreased hemoglobin level

5.lab test

Complete blood Count will help determine hematocrit,low hb,low platelet count

Coagulation profile will show INR, prothrombin time, bleeding time ,clotting factors

Liver profile will show deranged liver enzymes,lDH,billirubin,higher glucose levels

6.Diagnostic test

Blood laboratory workup

Ultrasound or MRI for suspected liver dysfunction.

EMG for checking seizure activity in case of Neuro involvement

7.safety consideration

The imidiate safety concern is to deliver the baby is the gestation week is above 34 weeks to prevent intrauterine death

If not completed 34 weeks care should be taken for regular monitoring of fetal growth,blood pressure,blood work up

8.Health promotion

HeLLp can be prevented by maintaing a healthy weight,blood pressure blood glucose by consuming a low carb and high protein diet,and by regular exercise.

Mothers at higher risk of HELLP syndrome with hypertension,diabetes,previous eclampsia should consult a obstretician before planning next pregnancy
