Diagnosis: Tachycardia, hypertension, delusions, hallucinations, seizures, hyperthermia, chest pain.

Pathophysiology: Compulsive use of cocaine leads to dehydration, sleep and food deprivation. prolonged use increases physical tolerance thus, increased intake of cocaine causes toxicity which includes alterations in respiratory, cardiac, urinary systems functioning and hyperthermia.

Health promotion and disease prevention: The only way to prevent cocaine toxicity is to stop using cocaine. Community Reinforcement And Family Training (CRAFT) to deal with patients effected with cocaine toxicity are recently recommended by various researchers.

Risk factors: Psychological factors like anxiety, stress and depression. Environmental factors like peer and family substance use.Genetic predisposition. Starting alcohol or nicotine at early age.

Expected findings: Weight loss, excitability, dilated pupils, mood swings, nose bleed, social isolation, poor hygine habits, burn marks on hands and lips.

Laboratory tests: ABG analysis, Complete Blood Count, electrolyte, blood urea nitrogen (BUN), serom troponin, serum glucose, pregnancy test

Diagnostic procedures: ECG, CT scan, chest x-ray

Safety considerations: Physical restraints should be avoided due to risk of rhabdomyolysis. Caution is appropriate as patient maybe disoriented and aggressive.

Nursing care: Assess patient airway, breathing and circulation, administer oxygen if needed, Monitor vital signs including temperature, monitor patient blood glucose level beside. Administer medications as prescribed.

Therapeutic procedures: The therapeutic procedures include tests, medication for symptomatic treatment, harm prevention, cold compress, cardiac evaluation and psychiatric evaluation.

Medications: Symptomatic treatment is given based on patients diagnosis. Vasopressin in cardiac arrest, nitroglycerine in hypertension, norepinephrine in hypotension, thaimine in hypoglycemia. Medications to fight addiction are Topiramate (a headache and anti-seizure medication), Modafinil (a psychostimulant that affects dopamine release) and Baclofen (a muscle relaxant).

Client education: Client and family education is vital to encourage deaddiction to substance abuse. Educating on CRAFT and medications to fight addiction are highly suggested.

Interprofessional care: Caution should be taken while dealing with aggressive patients to prevent harm.

Complications: Suicides, accidents, homicide, polysubstance use.
