--EHR--it is a systematised collection of patient and population electronically stores health information in a digital format.
--Chronic care--refers to medical care and it addresses pre existing or long term illness as opposed to acute care which is concerned with short term or severe illness in brief duration
--offering diagnostic test alone does not mean to provide full care to the pt
hence in the aspect of meaning itself we can say that continuum of care provides full care to population and patient
first given 3 options---to separate physical and mental health,to readmit patients,to adopt new payment methods do not apply in the purpose of adopting continuum of care
