When 250mg=1 capsule

Then 2000mg=??

Cross multiply and divide

=2000×1÷250

=8 capsule/day in two divided doses mean

8÷2=4

Weight:110lb (0.454×110=49.9 or 50kg)

Medication :Fentanyl

Dose:2.5mg/kg

As the patient weights 50kg multiply by 2.5kg to get the dose

=125mg (50×2.5)

Weight:36pound (16.3 or 16kg)

Route:oral
