Answer: One of the cause of chronic disease is lifestyle of the patients. It can be explained by the example such as Obesity has become one of the most common problem and is always overlooked by the people. They don't give much concerned regarding increased weight. But the outcomes of the obesity is the chronic disease such as cardiovascular disease, diabetes, hypertension etc If person changes his life style and increase self care this will reduce the risk of chronic diseases as by maintaining normal weight.

Education provided to the patient regarding the diagnosis, treatment and the medication provided can improve the efficacy of the drug as well as it will lead to increase the positive outcomes. It creates safe environment for the patient and prevent from the side effects or the drug interactions.

Toh help low literacy client to learn essential health information many things or methods can be implemented such as:

• Using simple words while communicating with the clients.
• Intervene the level of understanding.
• Provide written as well as verbal information to the patient.
• If possible, explain with the help of diagrams or pictures so that the ability of understanding can be improved.
