The patient notes is very important document for planning and implementation of a work.The SOAP note or tool can be used her

They should be described the following way

Subjective:This should include any pain,irritation, discomfort said by the patient about the wound

Objective:note down the color,size,degree or stage of wound ,redness, oozing ,class,etc

Assessment:The patient has a wound on the sacrum

Plan:Alliginate dressing ( select the type of woundcdressng (hydrocolloid, hydrogel,alliginate and collagen dressing)

Alliginate method of dressing can be used to pack wound because it enhances hemeostasis, biodegradable, maintains adequate moisture by absorbing fluids or excess drainage.
