Answer #1

Lace score ranges from 1-19, and it also predicts the rate of readmission or death within around 30 days of discharge.

Hospital readmissions are very common nowadays. They are common, expensive and a key target for the medical care program. This evaluates the utility of the hospital readmission so that it can help to reduce the rate of readmissions in the hospitals.

The Lace in hospitals helps the hospital staff to know the patient's illness. Many patients got discharged from the hospital, but from them, only many patients readmitted within 30 days of discharge.

This shows that they are not treated nicely in hospitals. And because of this, health care organisations are performing there best to reduce the rate of readmissions in the hospitals.

They do every possible bit Soo that discharged patients shouldn't be coming back for the treatment. They are doing therapy with full concentration, and the patients are fully checkup while giving them discharge. This will help in reducing readmissions in hospitals.
