The answer is option 3 when a dna polymerase inserts wrong nucleotide during replication.

DNA of every organism has a proper sequence which if gets altered due to deletion of any nucleotide, insertions of wrong nucleotide or substitution with any wrong nucleotide causes changes in the DNA sequence which leads to mutations.
