A. Established Patient Preventive Medicine Services (CPT Code 99391- 99397): These codes are used for comprehensive care provided to patients beginning with infancy through patients age 65 and over. It includes periodic and preventive evaluations that reflect the age and gender appropriate history and examination.

B. Subsequent nursing facility care (CPT Codes 99307-99310): These codes are used for consults performed on a patient who has been provided with initial care, and reports to evaluate the patient further throughout the nursing facility stay.

C. New Office visit (CPT Code 99201-99205): These codes are used for consults performed on a new patient in the office. The patient should not have reported by a physician in the same specialty for at least three years.

D. Initial Observation Care (CPT Code 99218-99220): These codes are used for consults performed on patients designated or admitted under observation status.

E. Office consultation (CPT Code 99241-99245): These codes are used for consults performed on patients under observation status which is provided by a physician whose opinion or advice is requested by another physician.
