Sure, the client can have a conversation with the trainer about the injury but the first thing the trainer should do in this situation is to refer the client to a physical therapist or a doctor.

Why other options are incorrect

Checking out the issue (The trainer can check out the issue and talk to the client about certain possibilities and the trainer himself who trains the client should go talk to the client and not let some other trainer check out the client. The trainers should take responsibility for their own clients at least to some extent)
