Answer: Case management is a process in which different options and services required for meeting individual‘s health needs are assessed, planned, implemented, coordinated, monitored and evaluated.   This is a professional and collaborated process and assessment plays a vital role in it. Assessment is the first function that is carried out that initiates this process. This function involves referring the client to treatment based on the information gathered during the intake stage of the patient. In this stage, the case managers identify the patients problems, interests and risks involved in the treatment. Thus if anything goes wrong at this stage, the later stages automatically will become faulty. Without a good assessment proper treatment option for the patient cannot be established and if the right option is not established then it may result either patient refusing the treatment or getting a wrong one that is not according to his requirements. This makes it evident that a good assessment process is vital for the case management process.
