CREATE NURSING CARE PLAN :

Infant Case Study: You are babysitting twin 9-month old siblings; you have been caring for them for approximately 3
hours when they both gradually become irritable. This then transitions to non-stop crying over the span
of 30 minutes.

Diagnosis

  • Risk for Disorganized Infant Behavior
    • Which would mean that the infants are displaying alterations in heart rate, respirations, color changes, erratic body movements, difficulty with feedings or prolonged periods of wakefulness. (Central Lakes Medical, 2014).
  • Ineffective Infant Feeding Pattern
    • This was developed to name the infant's inability to suck or coordinate the suck/swallow response resulting in inadequate oral nutrition for metabolic needs. (Jalil, 2017).

Building off of the Irritable Infants Case Study , make an attempt at creating a nursing care plan for the situation. Include the following:

  1. include appropriate interventions for the infants that will assist in attaining the goal and expected outcomes originally noted
  2. hypothesize implementation and tolerance of the actions to arrive at hypothesized evaluation; note what evaluation data would indicate a discontinuation, revision, or modification of the plan of care
  3. identify what type of nurse care delivery is represented in the care of these two infants. How do you know?
  4. if in the acute care setting, what actions could you delegate? Support these decisions.

Submit your care plan (Word) and a short, typed summary of points 2-4. Again, the goal of this assignment is not the "perfection" of your care plan and analysis, but rather to demonstrate a basic knowledge of the nursing process and an attempt at applying it within a given scenario.

Diagnosis: Risk for diagnosed infant behaviour.

Goal:Reducing of Infant irritation & improving of normal behaviour of the Infant.

Interventions:1) Assess the general condition of the infant.2) Monitoring of vital signs like Heart rate,respiration(Tachyapnea,bradyapnea,apnea,& dysnea) 3) Observe the baby for all the reflexes mainly on Swallowing & Sucking reflexes.4) Assess for feeding pattern of the baby.5) Observe the color changes & skin tone of the baby.6) Assess for any neurological defects & any abnormal movements in a baby.7)Provide psychological support for parents.8) Assess for APGAR score & observe the phyiscal status of the baby.9)Observe for cleft lip & cleft palate disorders. 10) assess for the passage of urination & bowel pattern.11) Observe the bowel color, skin turgor, & all the reflexes.
