1.The nature of the procedure
we should explain the whole procedures to the client which includes the date of the procedure ,duration ,pre anaesthetic check up,type of anaesthesia, equipment ,the entire procedure in a simple manner so on.
2.The risk and benefits of the procedure
It is vital to discuss the patients about the risk of the procedure(risk or complications during procedure,post operative risks,chance of getting other problems,success rate )and benefits(complete recovery of the disease, increased life time,chance releaving symptoms,)
3.Reasonable alternatives
All the procedures have some other alternatives or other way to treat disease like medication ,rest ,and or exercises,ayurveda treatment ,yunani etc.That particular procedure may be not necessary for that particular disease in that patient..so we should inform all other alternatives before taking a consent.This makes patient to take proper decision.
4.Risk and benefits of alternatives
Along with provide reasonable alternatives,it is our responsibility to disclose all the risk and benefits of that procedure.
5.Assessment of the patient's understanding of elements one through four
Make sure that patient got clear image of that particular procedure and its risks ,benefits through asking some questions about that procedure.

Informed consent is a process in which a health care provider educates a patient about the risks ,benefits,and alternatives of a given procedure or intervention.The patient must be competent to make a voluntary decision about whether to undergo the said procedure.
