  • The client will exhibit increased comfort and relaxed muscle tone for body posture.

interventions and rationale

  • Assess the client’s pain, including the location, characteristics, onset, duration, frequency, quality, intensity, and severity - can find baseline data
  • Instruct the client that meals should eat at regular intervals in a relaxed setting -An irregular schedule of meals may interfere with the administration of medications.

Imbalanced nutrition less than body requirements related to nausea and vomiting as evidenced by weight loss

the client will verbalize and demonstrate the selection of foods or meals that will achieve a cessation of weight loss.

  • assess body weight- weight loss is an indication of inadequate nutritional intake.
  • Assist the client to avoid foods that cause gastric irritation - the client would increase food intake if the foods are not associated with pain

3. Anxiety related to fear of unknown as evidenced by alecory stimuli by maintaining a calm and quiet environment -Anxiety may increase to a panic state with excessive conversation, noise, and equipment around the client.

  • Assess blood glucose level before meals and at bedtime -Blood glucose should be between 140 to 180 mg/dL.
  • Assess for anxiety, tremors, and slurred speech -These are signs of hypoglycemia
  • Assess feet for temperature, pulses, colour, and sensation -to monitor peripheral perfusion and neuropathy

2. Deficient knowledge related unfamiliarity with insulin injection as evidenced by statements of concern.

  • Explain that regular prandial insulins (Humulin) should be injected 30 mins before meals and rapid-acting insulins (Novolog, Humalog) may be injected before or after eating -Dosage should be adjusted based on the actual amount of food ingested because rapid-acting insulins can be given after a meal
  • .Teach patient to rotate insulin injection site - multiple injections in the same site may cause fat deposits.
