Dear [Name],
On 15th September, we placed an order which have a shippment number is 5930. I want to draw your attention towards the important matters regarding this shippment.
First of all, the due delivery date for this shippment was 14th October 2019 but I received the order on 26th October 2019 due to which there was disruption in our workflow and our customers faced huge amount of inconvenience.
Your team failed to inform us about the delay.

Secondly, even after the delay, the products received are not as per the order placed. Out of 1000 items, 300 were not up to the mark.
I hope you agree about the inconvenience we faced because of your leniency. We didn't expect such unprofessionalism from a reputed and well established company like yours.

We request you to please look into this matter and replace the wrong products sent. Also, we want you to arrange proper compensation of the loss we incurred as a result of [company name] unsatisfactory service.

Sincerely,
[Signature]
