6. Mr. Black, age 89, has multi-infarct dementia with severe memory loss, impaired judgment, and mood swings. He is also profoundly deaf and has some vision problems. He is increasingly weak and unsteady on his feet and needs to be supervised and assisted with all activities of daily living. He lives in a private room in a long-term-care facility. He will not join in any activities and often refuses to go to the dining room for his meals. Sometimes he has angry outbursts and is often found crying in his room. He says that he should be able to go out and have a job again. He feels that everything has been taken away from him. He often expresses that his life is worthless and he wishes he could die. Mrs. Black visits her husband every day and attempts to rationalize with him and talk him out of his bad moods. She appears to care for him very much and often tidies his room and changes his clothes. He usually unloads all of his feelings on her and she goes away feeling frustrated and guilty.

Multi-infarct dementia is caused by a series of small stokes, with multiple areas of the brain are dead and there is a memory impairment and altered thought process.

a.In the present scenario.Mr.Black 89 years old has suffered from multi-infarct dementia with severe memory loss, impaired judgment, and mood swings. He is also profoundly deaf and has some vision problems. Because of these problems and he lives in a private room in a long-term-care facility. Because of these, all he is worried so much and he is not taking his diet properly and he is physically weak and he is thinking that his life is worst, this totally affects his mental and emotional health because of these all he came to the stage of depression.

b. Because of staying in a long-term care facility and in a private room, he is not cooperating with anyone in the facility, he refuses to take his meals and he is every time thinking that his life is worst and he wishes to die, Here we can see that because of institutionalization and his loss he came to the stage of depression and anxiety.

c.One of the maintain advantage from Mrs.Black visit is, Mr. Black, can express all his feelings and his bad moods to Mrs.Black. This can help to unload the negative feelings from his mind, and as well as Mrs.Black appears to care for him very much and often he changes his clothes and clean the room

d. One of the disadvantages of the Mrs.Black visit is that, during each and everyday day visit to the long-term care facility to see her husband Mr.Black, he express all his negative feelings, bad moods, and often expresses that his life is worthless and he wishes to die. Because of this while going away from the long term care facility after met her husband, she is emotionally weak and feeling frustrated and guilty. After Mrs.went again he will be alone in the private room in the long-term care facility and started worrying regarding his condition and situation.
