The dental hygiene care plan should be a vehicle for care that is safe, evidence- based, clinically sound, high-quality, and equitable. The plan should be personalized according to the individual's unique oral health needs, general health status, values, expectations, and abilities. When formulating the plan, dental hygienists should be sensitive and responsive to the patient's culture, age, gender, language, and learning style. They should demonstrate respect and compassion for individual patient choices and priorities.

There are total 6 standars of ADHA clinical practise. They are as follows. 1) Assessment 2) dental hygiene diagnosis 3) planning 4) implementation 5) evaluation 6) documentation....

Now we are going to discuss about first standard that is the assessment part. As this is the base part to carry on the further process let us have a deep discussion about it....

Its is the process of collecting dental data from the patient in order render proper care.there are several history back ground which has to be collected .. these are as follows

1). Health history.... in this family history, physical characteristics, social history, and medical history is to be collected.

2) clinical assessment : complete oral assessment must be done.gums must be examined , teeth alignment hast to be checked. Condition of the teeth must be assessed . For example, check for dental carry, tooth decay, foul smell etc.

3) Risk assessment: the person must be assessed for personal habits like food habits, tobacco usage etc. Family history must be collected regarding oral problems...

These are all the steps which is done in assessment part. Now let us discuss how is it related to other standards.

After collecting the health information the then if is base for  making diagnosis, planning of treatment must be done according to his life history and health history, implementation is done according to his condition and it must suit his socio economic life too which is assessed in assessment process ... followed by evaluation... hence this  is detailed explanation  about one of the ADHA dental standards
